Adult Day Health Centers (ADHC)/Community-Based Adult Services (CBAS)

All ADHC/CBAS are required to provide the following, based on individual participant’s needs: skilled nursing care; personal care; physical, occupational, speech therapy; mental health services; therapeutic activities; medical social work; hot meals and nutritional counseling; transportation to and from participant's residence.

Program of All-Inclusive Care for the Elderly (PACE) provides comprehensive medical/social service delivery system. To be eligible, person must be age 55+, reside in a PACE service area, be determined eligible at nursing home level of care by Department of Health Care Services (DHCS), and be able to live safely in their home or community at the time of enrollment. https://www.dhcs.ca.gov/services/ltc/Pages/programofall-inclusivecarefortheelderly.aspx
Medicare & Medi-Cal reimburse PACE providers directly for care. If enrolled in Medicare + Medi-Cal, pay nothing or pay Medi-Cal “share of cost” to PACE provider. PACE providers accept private pay.

Seniors Program connects eligible seniors age 65+ with local volunteer ophthalmologists who provide a medical eye exam often at no out-of-pocket cost, and up to one year of follow-up care for any condition diagnosed during the initial exam, for the physician services. Eligibility: U.S. citizen or legal resident; age 65+; not belong to an HMO or have eye care benefits through the VA; not seen an ophthalmologist in three or more years

Glaucoma Program provides a glaucoma eye exam at no cost to those who are eligible and uninsured. Those who are eligible and insured are billed and are responsible for any co-payments. (This is an awareness program to provide a baseline glaucoma eye exam to those who may not be aware they are at increased risk). Eligibility: U.S. citizen or legal resident; not belong to an HMO or have eye care benefits through the VA; not had an eye exam in 12 months or more; at increased risk for glaucoma, determined by your age, race and family history.

New Eyes https://new-eyes.org/application purchases new prescription eyeglasses for low-income individuals in the U.S. through an e-voucher program. Eligibility: Be in financial need - reside in a household with net monthly income below 250% of FPL; Have had a recent eye exam (within in last 24 months) with a pupillary distance measurement (or click here to learn how to measure pupillary distance). If you have a prescription older than 24 months, you may be able to renew your prescription available through our online application.
OneSight https://onesight.org/faqs 501(c)(3) non-profit agencies can apply on behalf of individuals in need of glasses by issuing a referral letter (on agency letterhead and include Tax ID# for nonprofit) verifying visual and financial need. Applicant takes referral letter + valid prescription to participating Luxottica Retail Location (LensCrafters, Target Optical or Pearle Vision corporate store) to receive a pair of glasses at no charge. If applicant patient does not already have a valid prescription (< 2 years old), applicant can ask if the onsite doctor at the optical retailer is able to donate an eye exam or they can reach out to Prevent Blindness for assistance.
